We endeavour to actively fundraise through events, projects, appeals and donations to support the Children, Young People and their families across the teamHDFT Footprint. By supporting the Great Start in Life Foundation, you can make a difference to the lives of Children and Young People across County Durham, Darlington, Gateshead, Middlesbrough, Northumberland, North Yorkshire, Stockton-on-Tees, Sunderland and Wakefield.

Our Services

The 0-19 (25 for Special, Educational Needs and Disabilities (SEND)) team are located in all localities across Northumberland. The service is predominantly a Home Visiting Service but contacts can be arranged in Family Hubs, colleges, education and community settings when required. The 0-19 Service works closely with all partner organisations who provide services for Children, Young People and families to provide positive outcomes for all. The Great Start in Life Foundation will help Children, Young People and families throughout Northumberland to enhance the Health Visitor and School Nursing services we provide, to ensure the Children and Young People who access these services have a Great Start in Life. We fund specialist equipment, training and services, going above and beyond the provision of the services for Children, Young People and their families to help enhance treatment and facilities for all within Northumberland.

Our work benefits patients, their families and our staff throughout the local area and we rely on the support of the community to raise funds. From “Stepping Up for GSIL” by taking on a marathon to having a local tea, coffee and cake morning, every effort you make will have an impact on the Children, Young People and their families across Northumberland. Through your support, we have been able to distribute care packages to colleagues, enhance the environment and experiences of service users as well as distribute lots of fantastic Sparkle Awards to our #teamHDFT colleagues and Volunteers that have gone above and beyond for our wonderful service users.

Gateshead

Our Services

The Growing Healthy Gateshead 0-19 Healthy Child Programme (HCP) Service is an integrated service for families, expectant mothers, Children and Young People that offers interventions from the pre-natal stage up to the age of 19.

TeamHDFT leads on the delivery of this service, with a focus on working across services and organisational boundaries to improve public health outcomes.

In Gateshead, we provide a Family Nurse Partnership and HENRY Service alongside our 0-5 Health Visiting Service and 5-19 School Nursing Service. For further information about the specific services that can be accessed in your area, please click here.

Our Services

Growing Healthy Sunderland provides an integrated Public Health Service for expectant mothers, Children and Young People and their families in the city. The team delivers high quality, evidence-based interventions which support families and identify and respond appropriately to need across the city of Sunderland. Our Services

The 0-25 Team are co-located in the Family Hubs and Children’s and Young People’s buildings across County Durham, integrated with partner organisations who provide services for Children, Young People and families. The service is community based, with appointments and visits offered in homes, Family Hubs, colleges, education and community settings.

In County Durham, we have the Growing Heathy Perinatal and Infant Mental Health (PIMH) Service, Healthy Child Programme (HCP) as well as some more common areas of information and support for 0-19(25) year olds. Darlington

Our Services

In Darlington, we provide the Healthy Child Programme (HCP) alongside our 5-19 Healthy Child Service. The Teams work in collaboration with a range of health, social and educational organisations. The role of the 0-5 Service is to protect and promote the health and wellbeing of Children under five and their families. The 5-19 Team promotes and protects the health and wellbeing of all Children and Young People aged 5 to 19. All families with a child under 5 in Darlington have access to a Growing Healthy 0-5 Team consisting of health visitors, screeners, early year’s practitioners and infant feeding specialists. They provide expert health assessments, advice, support and interventions for babies, children and families.

Our Services

The North Yorkshire Healthy Child Service has 4 pillars: 0-6 Pillar, The Infant Feeding Pillar, Family Diet and Nutrition Pillar, The Emotional Health and Resilience Pillar and The Safeguarding and Children in Care Pillar with three levels of service available to all children.

All families with a child under 6 in North Yorkshire have access to a Growing Healthy 0-6 Team consisting of Health Visitors, Family Health Practitioners, Family Health Nurses and Infant Feeding Specialists.
